2023-2024 Season Medical Release/Registration Form Instructions:
Parents, please fill out ONE form for EACH of your children 2nd grade through 12th grade.
These forms are needed for your child to participate in any off-site activities. Fill out one form and then put it in the secretary’s mailbox, Lisa Scholtens (either in the copy room or church mailbox).
These forms will be kept in the church office and all the various group leaders will make copies of only their students to take with them on any group outing.
Make sure all information is entered for your child and then talk to your child about the rules of safety when participating in a group and have your child sign the Commitment by Student section on the bottom.
Please ensure that printed forms print all on one page on a full size 8 1/2 x 11 sheet of paper. Emailed copies must be able to be printed on a full page (pictures of the forms do not print well). Please contact Lisa if you need assistance with this.
